Recurrent Benign Paroxysmal Positional Verigo at Different Canals

Abstract
Background and Objectives: Benign paroxysmal positional vertigo (BPPV) is known to be treated with Epley or Barbecue maneuver. But the recurrent BPPV is relatively common and they do not always occur in the original site. The purpose of this study is to find out characteristics and the treatment efficacy of the recurrent BPPV at different site.

Materials and Methods: Retrospective reviews was performed for 335 patients who were diagnosed as BPPV and treated with canalith repositioning procedure (CRP) at Asan medical center from Oct. of 1997 to Feb. of 2002.

Results: Overall recurrence rate was 22% (78/335) and recurrence rate at different canals was 7.5% (27/355). Recurrent BPPV cases in lateral semicircular canal (LSC) whose original site was posterior semicircular canal (PSC) were 11 and conversion from PSC to LSC were 11 patients. In 5 patients, more than 2 canals were involved. Average number of CRP applied to these patients were higher than those in non-recurrent BPPV and recurrent BPPV patients in the same canals.

Conclusions: Recurrent BPPVs on the different semicircular canals were more difficult to manage. Close observation and careful application of CRP are needed to treat the recurrent BPPV patiens.
